15. Surgical Treatment Using Perforator Flap for Thorax Keloid

Introduction: The island flap was designed such that it could be
elevated by an internal mammary artery perforator flap located at the
5th or 6th rib to prevent any defect after excising the keloid in the
center of the thorax. After the operation, electron beam irradiation of
16Gy was used, and the result was good. Here, we describe two cases of
thorax keloid. Case 1: A 74-years-old woman had a huge and infected
thorax keloid that we excised under general anesthesia. The internal
mammary artery perforator flap on both sides of the keloid was elevated.
The size of the flap on the right side was 16 x 4.5cm and that on the
left side was 15 x 4cm. Case 2: A 64-year-old man had a small thorax
keloid that we excised under local anesthesia. The internal mammay
artery perforator flap on the left side of the keloid was elevated. The
flap size was 7.5 x 2.5cm. Conclusion: This is the first report of
surgical treatment for thorax keloid using the internal mammary artery
perforator flap.
